Cleanup:

- Fix bug about using an (unordered) set for index and primary key columns
- Non-private fields first (Checkstyle reports this)
- Comments are on a separate line (Checkstyle)
- Space before and after "=" (Eclipse reports this)
- Comment, specially comment fragements, are lowercase
